Okay, I've filled it in... Now what?
Once you have completed the worksheet, please send it via email to your CSM. They will then coordinate an initial meeting for you with one of our stellar Implementation Engineers (IE). Based upon the information you have provided, the IE will work with you to configure your DialSource interface and all of the desired automation behind it.
