We have identified an issue affecting users in some organizations where initial attempts to place a call is met with the "your call could not be completed as dialed" prompt. However, subsequent attempts to the number will call normally.
The dev team is actively working on a solution and updates will be posted here as they are available.
UPDATE - 10:42a PDT: We've narrowed the issue down to one particular carrier (which is why some attempts work as the call is being routed to a new carrier). We are working on switching them out with an alternate provider and are working with the network in question to resolve their issue.
RESOLUTION - 11:11a PDT: The root cause of the issue has been determined. There was an issue with our provider's telephony switches that caused the call request to time out, this was interpreted by our system as a failed attempt. Our carrier has resolved this issue and calls are now routing normally.
